New feature – 2020 ICD-10 updates

Every year on October 1st, CMS (the Centers for Medicaid and Medicare Services) introduces several updates to the ICD-10 code set. IMO (Intelligent Medical Objects) automatically adds all of the new or updated ICD codes to their terminology, so you will be able to search for those new values on October 1st. But because CMS also ‘deletes’ some ICD-10 codes each year (and replaces them, typically, with more specific codes), the AngleSearch application is updated each year to automatically detect any expired (‘deleted’) codes and make them visually apparent to the provider. The expired codes appear with a light grey triangular warning icon. Should the provider wish to replace the expired ICD-10 code with a valid one, the icon can be clicked to bring up the search window with that problem in context, making it very easy to keep all of the ICD codes valid, and up to date.

AngleSearch has now been updated to detect all of the expired codes made by CMS from 2016 (when they first started expiring codes) up to the present (2020).

New feature – Edit problem from MEL

It has long been possible to launch AngleSearch using a MEL command that automatically triggers a specific search. For example, you can create a MEL command that launches AngleSearch so that “asthma” is automatically searched for. In that case, the “Find Problem” dialog will appear with “asthma” in the search field, saving the provider a few keystrokes. This can be useful when you want to partially automate the addition of a specific problem to the chart, but you don’t want to “hard-code” any ICD codes into the script command.

AngleSearch now supports the ability to launch AngleSearch with an existing problem in context, which automatically loads that problem into the “Change Problem” dialog. This can be useful for specialized clinical content where you know that a problem needs to be modified (perhaps due to a non-specific ICD code), but the provider needs to oversee the actual edit.

To launch AngleSearch with an existing problem in context, you will need to create a MEL script command like the following:

Note that the URL requires the PRID (the problem id) for the problem you wish to edit, which you can derive by parsing the output of the following MEL command:

The format of the MEL command is a little different for practices using older versions of Centricity (e.g. EMR 9.8 or CPS 12.0). For those products, the exact MEL command will depend on how your instance of AngleSearch was installed, so please contact AngleMD for assistance.

New feature – Custom ICD mapping

One of the great value propositions of using AngleSearch is the deep integration with IMO’s Problem IT terminology, which gives highly accurate and useful ICD-9 and ICD-10 mappings to the problem descriptions searched for by providers. There have been rare cases where a practice needs a different ICD code for a problem, however. Historically, we have supported using MEL trigger functions to handle events where a practice needs to deviate from IMO’s editorial decisions, but it isn’t always possible for a practice to edit the MEL functions that make up their clinical content. For those practices, we now support custom mapping within AngleSearch (configurable by practice) which allows for a practice to override any ICD code provided by IMO and translate it to one that would be more useful to your practice.

New feature – Stop date display

AngleSearch has always allowed each practice to display the last edit date or the last assess date for each problem (your choice), but you can now opt to show the stop date too. When this feature is activated, any problem that has a stop date will display that date directly below the problem description, alongside the problem comment (see image below). This makes it easy for providers to see at a glance which problems are going to drop off the problem list in the future.

New Feature – Age-related Rules

The whole point of AngleSearch is to make it easy for providers to find the best problem ICD-10 code for the patient without knowing how to be a professional coder. In practices that see pediatrics and adults, it can sometimes be difficult to know when special ICD-10 codes are required for children.

To help with this issue, AngleSearch now supports “Age-related Rules“. These “rules” are configurable and customer-specific. They can detect when the user is about to add an ICD-10 code that is inappropriate for the patient due to age considerations. If such a rule is triggered, the app will display a warning to the provider, and a button which links to a better search term. This makes it very easy for the provider to switch to the preferred term.

The system has detected that the user is trying to add the adult version of “UTI” to a 2-week old infant, and exposes a button which makes finding a better replacement easy.

New feature – Turn off the display of ICD-9 and SNOMED-CT codes in the Find Problem dialog

If your providers do not have a need to see the SNOMED-CT codes or the ICD-9 codes in the Find Problem dialog, those values can be toggled off so they no longer appear (either one, or both).

By default, the ICD-9 and SNOMED-CT codes appear in the Find Problem dialog, along with the ICD-10 code.

The ICD-9 code and SNOMED-CT codes can now be turned off.

Please just drop us a note if you would like to turn off the display of either code.

New feature – Display of formal ICD-10 description in the Find Problem dialog

AngleSearch users appreciate the clinician-friendly problem descriptions provided by the IMO terminology set we integrate with. While perfect for the EMR, those IMO problem descriptions do not flow over to the practice management system in some practices. Often, only the formal ICD-10 descriptions are used on the practice management side.

One practice has requested that AngleSearch render the formal ICD-10 problem description alongside the IMO problem description to help providers maintain awareness about what information will be flowing over to practice management. This optional feature was implemented in a way where if the ICD-10 description contains the word “unspecified”, that word will be highlighted so the providers can identify opportunities to make their diagnoses as clinically specific as possible. If the ICD-10 description does not contain the word “unspecified”, it is rendered in a lighter color so as not to be distracting.

New feature – Removing old medications quickly

As with the problem list, many practices have patient charts with a large number of medications that are no longer active and should be removed, but due to all the overhead involved with marking them as inactive and setting the reason for removal, they may just languish on the med list.

We’ve now added an optional menu to the medication list (exposed by clicking on a med) that bypasses the Remove Medication dialog and allows providers to complete a medication (i.e., change the med status to ‘inactive’ and set the reason to ‘Regimen complete’) with fewer clicks.  The goal is to make it super fast and easy for providers to remove all the old medications in the list just as soon as they are identified.

If a medication should be removed for any reason other than “Regimen complete”, then the provider can still access the old “Remove…” menu option, which will open the Remove Medication dialog, just as before.

Like all new user-facing features added to AngleSearch, this menu choice will only appear if your practice “opts in” to this feature.  This allows you to coordinate new features with any training that you might like to do.

Many practices have patient charts with a large number of problems that are no longer active and should be removed, but the providers often don’t have the time to invest in that kind of routine chart maintenance.  The more old problems that accumulate in the chart, the less likely they are to be removed due to all the overhead involved with marking them as inactive and setting the reason for removal.

AngleSearch has always included the ability to remove problems, but up until now, we’ve done so via the Centricity Remove Problem dialog.   Removing a problem, therefore, wasn’t necessarily any easier to do in AngleSearch as it was from the Update Problem dialog.

We’ve now added an optional menu to the problem list (exposed by clicking on a problem) that bypasses the Remove Problem dialog and allows providers to resolve a problem (i.e., change the problem status to ‘inactive’ and set the reason to ‘Resolved’) with fewer clicks.  The goal is to make it super fast and easy for providers to remove all the old problems in the list just as soon as they are identified.
